Five Rights Verification

Controlled drugs always require the full five-step check, regardless of this setting.

Controlled drugs — special rules

Controlled drugs (e.g., morphine, midazolam) must be witnessed during disposal and recorded in the CD register. Contact your pharmacy for guidance.

Active Titration Schedule

⚠ This medication follows an active titration schedule.

Dose changes made outside the schedule can be overwritten when the schedule advances to its next step. If the prescriber has changed the plan, edit the titration schedule itself.

What do you want to edit?

ⓘ Change one step only — for example, the GP extended step 2 from 7 to 14 days, or adjusted that step's dose. The rest of the schedule stays as it is.
ⓘ Restructure the plan — add or remove steps, or change the overall progression. Steps that have already been completed are kept as historical record and cannot be changed.
ⓘ Change things the schedule doesn't control — notes, route, form, PRN settings. Opens the normal edit form. Avoid changing the dose here while a titration schedule is running.

CONTROLLED DRUG — Schedule 2

This medicine is a controlled drug. CQC requirements:

  • Must be stored in a locked controlled drugs cupboard
  • Requires a separate CD register entry
  • Stock balance must be checked and recorded at every administration
  • Destruction must be witnessed by an authorised person
ⓘ Some medicines must be discarded within a set number of days after the bottle or tube is first opened, even if the printed expiry date is later. This is sometimes called the "shelf life" or "in-use expiry". Check the medicine label or ask the pharmacist for the correct number of days.

Common examples: Eye drops = 28 days, Oral liquids = 14-90 days. Check the medicine label.

ⓘ A titration schedule gradually adjusts the dose over time, as prescribed by the GP or specialist. Turn this on if the prescriber has set a step-by-step dose change plan (e.g., start on 100mg for 7 days, then increase to 200mg).
